New Rules for SG Child Vaccine Record Validation
2026-10-09
Major updates have been made to the authorities and procedures for Singapore vaccination record certification.
Foreign children under 12 in Singapore are required to submit certified vaccination records for school enrolment and passmate applications including DP, LTVP and STP. If your child received all vaccinations in China, you will need to convert the records to meet Singapore’s official standards. Below is a step-by-step guide to the application.
Step 1: Review Vaccination Records & Check Catch-up Requirements
Focus on two core vaccine types to verify compliance with Singapore’s schedule:
- DPT (Diphtheria, Pertussis, Tetanus), known as the 3-in-1 DTaP vaccine in China
- Measles vaccine
Under Singapore’s National Childhood Immunisation Schedule, children receive 2 doses of MMR (measles) vaccine at 12 months and 15 months of age.
The measles vaccination timeline differs in China. The first dose in China is normally administered between 8 months and 1 year old. Singapore’s Ministry of Health does not recognise measles shots given before 12 months of age.
It is critical to assess whether catch-up measles vaccination is required before certification. Reach out to our team if you need professional assessment.
Step 2: Book Clinic Appointment for Catch-up Vaccination
If your child fails to meet the vaccination criteria, book an appointment at a local clinic for outstanding doses. Request the doctor to complete the CDA form, downloadable from the Communicable Diseases Authority (CDA) website.
Step 3: Complete CDA Form by a Registered Doctor
If your child already satisfies all vaccine requirements, send supporting documents to a doctor for CDA form completion.
A key change: the official body for vaccination record certification has shifted from HPB (Health Promotion Board) to CDA (Communicable Diseases Authority).
Prepare translated copies of the original vaccination booklet for the doctor.
Our team can assist with CDA form downloads, vaccine booklet translation, clinic booking for catch-up jabs and doctor consultations for CDA form filling.
Step 4: Upload All Documents
Once you obtain the completed CDA form and other supporting paperwork, submit everything via the CDA portal.
All non-English documents must be translated. Contact us if you need help with online submission.
Step 5: Receive Notification from CDA
You will normally get an email update within 10 working days after submission. Once approved, log into the CDA portal to download your certified vaccination document.
This covers the full workflow for Singapore vaccination record certification.
